To apply for therapy services with the Vermont Center for Anxiety Care:

1. Complete an initial intake form first.​ Only one form needs to be submitted for a Family (family members together) or Couple inquiry under whose insurance, sessions will be billed to.

​2. ​Completing your therapist's additional forms *online* and *before* your first scheduled session facilitates efficiency and ease in starting care. Once an appointment is scheduled with a therapist please complete the additional forms of your therapist below, anytime before your first session.
